VILPRAMYCIN SAR® is an oval, biconvex tablet with a risk on one side, almost white or white with a yellowish tinge or yellow color, with a strawberry smell.
The active ingredient is josamycin.
Each dispersible tablet contains 1000 mg of josamycin (in the form of propionate).

Recommended dose
Adults
The recommended daily dosage for adults is from 1000 to 2000 mg
Josamycin. The daily dose should be divided into 2-3 doses. If necessary, the dose can be increased to 3,000 mg per day.
Use in children and adolescents
Children and adolescents aged 14-18
The dosage regimen for children from 14 to 18 years of age does not differ from the dosage regimen for
adults.
Children aged from 1 year with an average body weight of at least 10 kg and up to 14 years
Children aged 1 year have an average body weight of 10 kg. The daily dosage for children with a body weight of at least 10 kg is prescribed based on the calculation of 40-50 mg / kg of body weight daily, divided into 2-3 doses:

    for children with a body weight of 10-20 kg, the drug is prescribed 250-500 mg 2 times a day (1/2 tablet dissolved in 50 ml of water). If the drug is prescribed at a dose of 500 mg, the resulting suspension should be taken in full. If the drug is prescribed at a dose of 250 mg, the resulting suspension of 1/2 tablet is divided into two doses (25 ml per dose), the second half of the suspension is stored at a temperature of 2 to 8 ° C until the next dose, but not more than 12 hours. The shelf life of the resulting suspension should not exceed 12 hours). For children with a body weight of 20-40 kg, the drug is prescribed 500-1000 mg (1/2 tablet – 1 tablet dissolved in 50 ml of water) 2 times a day, over 40 kg – 1000 mg (1 tablet dissolved in 50 ml of water) 2 times a day.

Children under the age of 1 year with a body weight of less than 10 kg
VILPRAMYCIN SAR® is not used in children under the age of 1 year with a body weight of less than 10 kg due to the inability to provide a dosage regimen.

Route and/or method of administration
Inside.
Immediately before taking the tablet, it should be dissolved in a small amount of water (at least 50 ml) and mixed thoroughly until a suspension is obtained. The resulting suspension must be taken immediately after preparation or the remainder of the suspension must be stored at a temperature of 2 to 8 ° C until the next intake, if the finished suspension is divided into two doses. The tablet can be divided into equal doses.

Josamycin is active against Gram-positive bacteria and Gram-negative bacteria. The drug disrupts protein synthesis in the microbial cell, thereby slowing down the growth and reproduction of microorganisms sensitive to it. Josamycin has a drug effect at a fairly high rate 1 hour after ingestion of the drug.

The drug VILPRAMYCIN SAR® is used in adults and children aged from 1 year with a body weight of at least 10 kg for the treatment of infectious and inflammatory diseases,
caused by microorganisms sensitive to the drug:

  • upper respiratory tract and ENT infections: tonsillitis, pharyngitis, paratonsillitis, laryngitis, otitis media, sinusitis, diphtheria (in addition to treatment with diphtheria toxoid), scarlet fever (in case of hypersensitivity to penicillin);
  • lower respiratory tract infections: acute bronchitis, exacerbation of chronic bronchitis, community-acquired pneumonia, including those caused by atypical pathogens, whooping cough, psittacosis;
  • infections in dentistry: gingivitis, pericoronitis, periodontitis, alveolitis, alveolar abscess;
  • infections in ophthalmology: blepharitis, dacryocystitis;
  • infections of the skin and soft tissues: folliculitis, furuncle, furunculosis, abscess, anthrax, erysipelas, acne, lymphangitis, lymphadenitis, phlegmon, panaritium, wound (including postoperative) and burn infections;
  • infections of the genitourinary system: urethritis, cervicitis, epididymitis, prostatitis caused by chlamydia and/or mycoplasma, gonorrhea, syphilis (with hypersensitivity to penicillin), venereal lymphogranuloma;
  • diseases of the gastrointestinal tract associated with Helicobacter pylori: peptic ulcer of the stomach and duodenum, chronic gastritis and others.

To date, there is no data on the specific symptoms of overdose. In case of overdose, the symptoms described in the "Adverse reactions" section of the instructions should be expected, especially from the gastrointestinal tract.

Some adverse reactions can be serious and life-threatening. Tell your doctor immediately if you experience any of the following adverse reactions while taking VILPRAMYCIN SAR®:

  • vomiting, may occur infrequently;
  • diarrhea, may occur infrequently;
  • rapidly developing and increasing swelling of the skin, mucous membranes – occurs abruptly, develops spontaneously and resolves just as quickly (angioedema), may occur rarely;
  • fever, pain, redness and swelling of the oral mucosa, ulceration (stomatitis), may occur rarely;
  • fainting or loss of consciousness, impaired breathing, decreased blood pressure, pale or bluish, cold, moist skin (anaphylactic reactions), may occur rarely;
  • fever and "malaise", followed by a widespread rash on the skin and mucous membranes in the form of spots, blisters, weeping erosions, crusts (Stevens-Johnson syndrome), may occur very rarely;
  • skin rash or skin lesions with a pinkish red ring and pale center that may be itchy, scaly, or filled with fluid. The rash may appear more often on the palms or soles of the feet (erythema multiforme), may occur very rarely;
  • cramping abdominal pain due to deterioration of the general condition of the body, frequent stools (pseudomembranous colitis), may occur very rarely;
  • fever and "malaise", followed by a widespread rash on the skin and mucous membranes in the form of spots, blisters, weeping erosions, crusts, skin detachment (toxic epidermal necrolysis), the frequency is unknown.

Do not take VILPRAMYCIN SAR®.:

  • if you or your child are allergic to josamycin or other macrolide antibiotics, or any other components of the drug (listed in section 6 of the leaflet);
  • if you or your child have severe liver dysfunction.

Tell your doctor that you are using, have recently used, or may start using any other medications.
Your doctor needs to know about the medications you are using to take into account possible adverse reactions that may develop due to the interaction of various medications with VILPRAMYCIN SAR®, and possibly adjust the doses of the medications you use.
