Solving the problem of resistance: Russian josamycin to be studied in real clinical practice

The biopharmaceutical company PROMOMED has initiated a medical study of a josamycin-based antibiotic under the trade name Vilpramycin® SAR. The study focuses on efficacy and safety in real-world clinical practice in patients with various concomitant diseases and a history of antibiotic use, and also allows to reach a wide population. PROMOMED Company conducts systematic research in real clinical practice, developing an evidence base and increasing confidence in domestic medicines among healthcare professionals.

Josamycin belongs to the group of macrolides. This is one of the safest classes of antibiotics. In clinical practice, the drug shows high efficacy against a number of intracellular microorganisms, which often cause infections resistant to standard antibacterial therapy.

The antibiotic Vilpramycin® CAP was registered in October 2025. Its full—cycle production - from the substance to the finished dosage form — is localized at the Biochemik plant in Saransk. The release of the antibiotic solved the problem of the shortage that arose after the withdrawal of the original drug Vilprafen Solutab from the Japanese company Astellas.

The domestic drug Vilpramycin® SAR is available in the form of dispersible tablets of 1000 mg. In the Russian market, this drug is presented as the only dispersible form of josamycin. This is especially convenient for children and patients for whom swallowing pills may be difficult. Additionally, it should be noted that josamycin is included in the clinical recommendations of the Ministry of Health of the Russian Federation, as well as in the updated clinical recommendations of 2025 for the treatment of community-acquired pneumonia in children.

Josamycin is used in the treatment of diseases of the respiratory tract, ENT organs, as well as infections of the skin, mucous membranes, organs of vision and the genitourinary system. One of the key advantages of josamycin is considered to have a high safety profile: its use is acceptable during pregnancy and breastfeeding, as well as in pediatric practice.1
