Always use the drug in full accordance with the recommendations of the attending physician. If in doubt, consult your doctor.
Your treatment with Pemetrexed Promomed was prescribed by a doctor who has experience in the use of drugs for the treatment of cancer. Your doctor will determine which dose of the drug you need and how often, based on your age and body surface area (body surface area is calculated based on height and body weight).
Recommended dose
The recommended dose of pemetrexed is 500 mg per square meter of your body surface. This dose may be adjusted or treatment may be delayed depending on the number of blood cells and your general condition.
The way and method of administration
You will always receive Pemetrexed Promomed as an intravenous infusion. The duration of the infusion is approximately 10 minutes.
When using the drug Pemetrexed-Promomed in combination with cisplatin:
The doctor will select the required dose depending on your height and weight. Cisplatin is also injected into one of your veins approximately 30 minutes after the end of the pemetrexed infusion. The duration of cisplatin infusion is approximately 2 hours. As a rule, you will receive therapy once every 3 weeks.
Additional medications for pemetrexed therapy
Glucocorticosteroids: Your doctor may prescribe you hormones in the form of tablets (equivalent to 4 mg of dexamethasone 2 times a day), which you will need to take the day before, on the day and the day after treatment with pemetrexed. This medicine is prescribed to reduce the frequency and severity of skin reactions that may occur during treatment.
Vitamins: Your doctor may prescribe folic acid (vitamin) or multivitamins containing folic acid (350 to 1,000 micrograms), which you should take once a day while you are taking pemetrexed. You should take at least 5 doses within seven days before the first dose of pemetrexed is administered. You should continue taking folic acid for 21 days after the last dose of pemetrexed. You will also receive an injection (injections) of vitamin B12 (1000 mcg) a week before taking pemetrexed, and then approximately every 9 weeks (which corresponds to 3 courses of treatment with pemetrexed). You are prescribed vitamin B12 and folic acid to reduce the possible toxic effects of antitumor treatment.

The drug Pemetrexed-Promomed is intended for the treatment of adult patients over the age of 18 with the following diseases:
Non-small cell lung cancer
The drug Pemetrexed-Prommed in combination with cisplatin (a drug used to treat a number of cancers) is indicated for the first-line therapy of patients with locally advanced or metastatic non-small cell lung cancer (malignant tumor), except for the predominantly squamous histological form (the diagnosis is determined by the results of a biopsy). Pemetrexed-Promomed as monotherapy (single drug treatment) is indicated for the maintenance therapy of locally advanced or metastatic non-small cell lung cancer, except for the predominantly squamous histological form, in people with no progression after the first line of platinum-based therapy.
The drug Pemetrexed-Promomed as monotherapy is indicated for second-line therapy in patients with locally advanced or metastatic non-small cell lung cancer, except for the predominantly squamous histological form.
Malignant mesothelioma (tumor) pleura (the serous membrane covering
the surface of the lungs)
The drug Pemetrexed-Prommed in combination with cisplatin is indicated for the treatment of people with unresectable (inoperable) malignant pleural mesothelioma who have not previously undergone chemotherapy.

Since the drug will be administered by a doctor, it is highly unlikely that you will receive more of the drug than you need.
If an accidental overdose occurs, the medical staff will closely monitor your condition and provide appropriate treatment.

Like all medications, Pemetrexed Promomed can cause undesirable reactions, but they do not occur in everyone.
Some adverse reactions can be serious and life-threatening. Tell your doctor immediately if you experience any of the following adverse reactions when using Pemetrexed Promomed:
− If you have a temperature of 38 °C or higher, sweating, or other signs of infection (because you may have fewer white blood cells than usual, which can be very common). Infection (sepsis) It can be severe and can lead to death.
− If you experience chest pain (infrequently) or palpitations (rarely).
− If you have pain, redness, swelling or ulcers in your mouth (inflammation of the mucous membrane, stomatitis - very often).
− If you develop a skin rash (very often)/skin itching (often) or fever (often) (allergic reaction). In rare cases, skin reactions can be severe and can lead to death. Consult your doctor if you have fever and "malaise", a common rash on the skin and mucous membranes in the form of spots, blisters, weeping erosions, crusts, peeling of skin areas (Stevens-Johnson syndrome and toxic epidermal necrolysis).
− If you feel tired, weak, short of breath, or you look pale (because you may have less hemoglobin (anemia) than usual, which can be very common).
− If you have bleeding from your gums, nose, or mouth, or any bleeding that doesn't stop, reddish or pinkish urine, unexpected bruising (because you may have fewer platelets than usual, which can be very common).
− If you experience sudden shortness of breath, severe chest pain, or coughing up blood in your sputum (pulmonary embolism is uncommon).

Do not use the drug Pemetrexed-Promomed:

  • if you are allergic to pemetrexed or any other components of the drug listed in section 6 of the leaflet;
  • if you are breastfeeding;
  • if simultaneous use of the drug with the vaccine is necessary
  • yellow fever.

Tell your doctor that you are taking, have recently taken, or may start taking any other medications, including over-the-counter medications and herbal remedies. This is important because the drug Pemetrexed-Promomed can enhance the adverse reactions of other drugs, affecting the liver and kidneys, which play an important role in the elimination of drugs from the body.
Concomitant use of: yellow fever vaccines is contraindicated due to the risk of complications.
Be sure to tell your doctor if you are taking or about to take any of the following medications at the same time:
− aminoglycosides (antimicrobial drugs);
− "loop" diuretics (diuretics);
− platinum-containing drugs (cancer drugs containing platinum);
− cyclosporine (a drug that suppresses the activity of the immune system);
− probenecid (a drug for the treatment of gout is a swelling or pain localized in the joint of the first finger); − penicillins (antimicrobials), because they can reduce the excretion of pemetrexed from the body;
− nonsteroidal anti-inflammatory drugs (medicines against fever, pain and/or inflammation), including ibuprofen, diclofenac, piroxicam and others (see the instruction leaflet); − drugs that reduce blood clotting, such as warfarin and others.
With their simultaneous use, there is an increased risk of blood clotting disorders
